Privacy Policy
FHC Consulting LLC ("FHC Consulting," "we," "us") respects your privacy. This policy explains what information we collect through fhchealthconsulting.com (the "Site"), how we use it, and the choices you have.
Information we collect
Information you give us. When you use our contact form or email us, we collect what you choose to send: your name, organization, work email, role, the topic you select, and your message.
Information collected automatically. Like most websites, our hosting and content delivery provider records basic technical data when you visit, such as your IP address, browser type, and the pages requested. This is used to deliver and secure the Site. We do not use advertising trackers, analytics cookies, or tracking pixels, and the Site itself does not set cookies. Our hosting provider may set a strictly necessary security cookie to protect the Site from abuse.
Please don't send patient information
The contact form and general email are not intended for protected health information (PHI) or any other sensitive health data. Please don't include patient names, record numbers, or clinical details. When an engagement requires access to PHI, we sign a Business Associate Agreement first and exchange that information only through secure methods agreed in writing.
How we use information
- To respond to your inquiry and schedule calls
- To prepare quotes, proposals, and statements of work you request
- To provide and manage our services if you become a client
- To operate, maintain, and secure the Site
- To comply with legal obligations and protect our rights
We do not sell or rent your personal information, and we do not share it for cross-context behavioral advertising. We will not add you to a marketing list without your permission.
Service providers we use
We share information only with providers that help us run the Site and our business, and only as needed for them to perform their services:
- Cloudflare, which hosts the Site and protects it from attacks
- Formspree, which processes contact form submissions and forwards them to our email
- Google Workspace, which provides our business email
- Google Fonts, which supplies the typefaces on the Site. Your browser requests them from Google, which receives your IP address.
We may also disclose information if required by law, to protect our rights or the safety of others, or as part of a business transfer such as a merger or sale.
How long we keep information
We keep inquiry information as long as needed to respond and follow up, and for client records as long as needed to provide services and meet legal, tax, and contractual requirements. We delete or de-identify information when it is no longer needed.
How we protect information
We use reasonable administrative, technical, and physical safeguards, including encrypted connections (HTTPS) and multi-factor authentication on our accounts. No method of transmission or storage is completely secure, so we cannot guarantee absolute security.
Your choices and rights
You can ask us to access, correct, or delete the personal information you have sent us, or to stop contacting you. Depending on where you live, you may have additional rights under state privacy law. To make a request, email us at the address below. We will respond within a reasonable time and may need to verify your identity. We will not treat you differently for making a request.
Children
The Site is intended for business professionals and is not directed to children under 13. We do not knowingly collect information from children.
Links to other websites
The Site may link to other websites. Their privacy practices are their own, and we encourage you to read their policies.
Changes to this policy
We may update this policy from time to time. When we do, we will change the effective date above. Significant changes will be noted on this page.
